Need help with your Newlife application?
We know filling out forms isn’t always the easiest task, so at Newlife we have tried to make the process as simple and straight forward as possible.
Application forms for all our equipment services – Equipment Grants, Emergency Equipment Loans and Play Therapy Pods – can be filled in and submitted online or downloaded if that’s an easier option for you, and there is also a booklet you can download about each of the service you can apply to, with lots of information which may be of help.
However, we realise that not everyone’s circumstances are the same, so if If you do run into a problem with your application there is always help on hand to advise you on the best way forward.
If you struggle to fill in any part of the application, find a professional to support your application, or would just like an update on the status of an application already made, then you can always turn to the Child and Family Service Team to help.
You can also talk to the team about your circumstances before filling in the form as they can advise you on anything you are unsure of, such as the best equipment to apply for to meet your child’s needs or whether you should be applying for an emergency equipment loan or equipment grant.
The Nurse Helpline is the best way to find help. Staffed by experienced and friendly nurses, who are also able to give you condition specific support, you can speak to them by calling them on the free Nurse helpline on 0800 902 0095, which is available Monday to Friday between 10am and 4.30pm.
